Yes. Any person may respond on behalf of a recipient who is unable to respond independently. When responding online, log in using the recipient's name, date of birth, and participant number as indicated on the Qualification Questionnaire. You will have the opportunity to explain that you are completing the Qualification Questionnaire on someone else's behalf.
